VA accepts lay evidence from the veteran or another person. A statement can describe an event, symptoms someone observed, or when a condition changed. The writer does not need medical training.
A lay statement should stay with first-hand facts. It should not offer a medical diagnosis or opinion the writer is not qualified to give. VA Form 21-10210 is one way to submit a lay or witness statement.
Identify records VA may help obtain
VA describes a duty to make reasonable efforts to help gather VA medical records, military service records, other federal records, and authorized private medical records. You still need to identify the records, the dates, and where VA can request them.
