Build an evidence-gap map

Select the evidence issues that show up in the decision letter or claim file. The worksheet returns records and questions to organize before a VA or accredited-representative discussion.

A witness, family member, service member, or the veteran can describe facts VA may not have discussed. The denial points to missing service treatment records, personnel records, locations, or dates. The file has symptoms or a diagnosis, but the decision questions the connection to service. A C&P exam, DBQ, or medical opinion may be missing, incomplete, or misunderstood. A Higher-Level Review decision still leaves an evidence issue unresolved. VA requested evidence, or the claim status tool shows new evidence under review.
